About Dr Fonseka
Dr Damitha Fonseka is a Sri Lankan Consultant Vascular and Transplant Surgeon with extensive postgraduate surgical training and advanced international experience in the management of complex vascular disease, vascular emergencies, vascular trauma, diabetic foot disease, chronic wounds, venous disease and renal transplantation.
Following his specialist surgical training in Sri Lanka, Dr Fonseka undertook advanced overseas training in vascular surgery at Addenbrooke’s Hospital, Cambridge University Hospitals NHS Foundation Trust, United Kingdom, one of the UK's leading tertiary academic and vascular centres.
During his time in Cambridge, he worked as a Senior Clinical Fellow in Vascular Surgery within a high-volume multidisciplinary vascular service. His experience included the assessment and operative management of complex arterial and venous disease and exposure to both open and endovascular vascular surgery, including abdominal aortic aneurysm repair, carotid endarterectomy, lower-limb revascularisation and bypass surgery, vascular emergencies and complex venous interventions. He was also actively involved in multidisciplinary team meetings, morbidity and mortality meetings, clinical audits, research and surgical education.
After completing his advanced training in Cambridge and returning to Sri Lanka, Dr Fonseka joined the National Hospital Galle, where he works as a Consultant Vascular and Transplant Surgeon, providing specialist vascular and transplant surgical care to patients with a wide spectrum of vascular conditions.
Extensive Operative & Surgical Experience
Dr Fonseka has developed substantial operative experience through years of intensive postgraduate surgical training, dedicated vascular and transplant training, and advanced fellowship-level experience in the United Kingdom.
His operative portfolio encompasses both elective and emergency vascular surgery, ranging from routine vascular procedures to major limb- and life-saving operations.
His experience includes:
-
Open abdominal aortic aneurysm repair
-
Endovascular aneurysm repair and thoracic endovascular procedures
-
Carotid endarterectomy
-
Lower-limb arterial bypass and complex revascularisation
-
Acute limb ischaemia and surgical embolectomy
-
Complex peripheral arterial reconstruction
-
Vascular trauma and emergency arterial/venous reconstruction
-
Limb salvage surgery
-
Endovenous laser ablation for varicose veins
-
Ultrasound-guided foam sclerotherapy
-
Complex venous disease
-
Arteriovenous fistula creation and complex dialysis access surgery
-
Aneurysm and pseudoaneurysm repair
-
Vascular malformations
-
Diabetic foot surgery and advanced wound management
-
Renal transplantation and transplant vascular reconstruction
His training has involved extensive exposure to open vascular techniques as well as endovascular procedures, supported by experience in vascular Duplex ultrasonography and perioperative vascular assessment.
Complex Vascular Reconstruction & Limb Salvage
A particular strength of Dr Fonseka's practice is the management of complex limb-threatening vascular disease and vascular trauma.
His experience includes challenging arterial injuries and limb-threatening emergencies requiring autologous vein graft reconstruction, arterial interposition, bypass surgery, embolectomy, fasciotomy and multidisciplinary limb reconstruction.
This background allows him to approach difficult vascular cases with a central objective:
Save the patient. Preserve the limb. Restore function whenever possible.
He has a particular interest in avoiding unnecessary major amputation through appropriate vascular imaging, revascularisation, infection control, wound management and multidisciplinary reconstruction.
Modern Varicose Vein Treatment
Dr Fonseka offers a contemporary approach to the management of varicose veins and chronic venous disease.
Where clinically appropriate, treatment has moved beyond conventional open varicose-vein surgery towards modern minimally invasive techniques, including:
Endovenous Laser Ablation (EVLA) • Ultrasound-Guided Foam Sclerotherapy • Duplex-Guided Venous Assessment
These techniques can allow appropriately selected patients to undergo treatment using minimally invasive approaches with rapid mobilisation and recovery.
Diabetic Foot, Chronic Wounds & Limb Preservation
Another major area of interest is the management of diabetic foot disease, chronic wounds and limb-threatening infection.
Rather than treating the wound in isolation, Dr Fonseka follows a vascular approach that considers the complete problem—arterial perfusion, infection, tissue viability, venous disease, pressure/off-loading and the patient's underlying medical condition.
Treatment may therefore range from specialist wound care and debridement to arterial reconstruction, bypass surgery and other limb-salvage procedures.
His postgraduate training has included dedicated exposure to wound management and complex vascular wound care.
Renal Transplantation & Dialysis Access
Dr Fonseka is trained not only in vascular surgery but also in transplant surgery.
His experience includes living and deceased-donor renal transplantation, vascular reconstruction during kidney transplantation and the management of complex transplant-related vascular anatomy.
He also performs arteriovenous fistula creation and vascular-access surgery for patients with chronic kidney disease, including assessment and management of complicated dialysis access.
This combination of vascular and transplant expertise provides an important advantage when managing patients with chronic kidney disease and complex vascular problems.
Broad Surgical Foundation
Before and during his specialist vascular training, Dr Fonseka gained experience across a broad spectrum of surgical disciplines, including general surgery, plastic and reconstructive surgery, cardiothoracic surgery, gastrointestinal surgery, trauma surgery, orthopaedic surgery, neurosurgery, urology and other surgical specialties.
He also has substantial previous exposure to anaesthesia, intensive care and resuscitation, providing a strong perioperative foundation for managing critically ill and high-risk surgical patients.
This multidisciplinary background is particularly valuable in vascular surgery, where complex patients frequently require expertise extending beyond an isolated blood-vessel procedure.
International Training. Contemporary Vascular Care
Dr Fonseka represents a new generation of extensively trained vascular surgeons in Sri Lanka, combining comprehensive Sri Lankan specialist surgical training with advanced experience from one of the United Kingdom's major academic vascular centres.
His practice incorporates contemporary approaches to vascular disease, including:
Open Vascular Surgery
Endovascular Techniques
Endovenous Laser Treatment
Ultrasound-Guided Interventions
Duplex-Based Vascular Assessment
Complex Limb Revascularisation
Modern Wound & Limb-Salvage Strategies
Renal Transplantation
His objective is not simply to introduce newer technology, but to use the most appropriate evidence-based treatment for each individual patient—whether that is medical management, minimally invasive intervention, open reconstruction or complex multidisciplinary surgery.
Academic, Research & Teaching
Alongside his clinical practice, Dr Fonseka has maintained a strong commitment to research, audit and surgical education.
He has undertaken research across important areas of vascular surgery and has presented academic work at national and international surgical meetings. His professional development has included involvement in clinical governance, morbidity and mortality meetings, vascular MDTs and surgical audit.
He has taught postgraduate surgical trainees, medical students, medical officers, nurses and other healthcare professionals and, during his Cambridge training, participated in medical-student education and as faculty in ASPIRE vascular surgical training courses for registrars.
A Patient-Centred Approach to Modern Vascular Surgery
For Dr Fonseka, excellence in vascular surgery means more than technical ability.
It requires accurate diagnosis, careful patient selection, sound clinical judgement, mastery of both traditional and modern techniques, multidisciplinary collaboration and compassionate communication with patients and their families.
His philosophy is to provide each patient with an individualised treatment strategy based on the nature of the disease, available evidence, operative risk and the patient's long-term quality of life.
Whether treating a patient with varicose veins or managing a complex aortic aneurysm, vascular trauma, diabetic limb-threatening disease or renal transplantation, the same principles guide his practice:
Precision. Compassion. Excellence.
